WHAT DATA DO WE STORE & HOW IS IT USED?
The Posture Practice will collect your name and basic contact details for the purpose of booking your sessions. You will also be required to fill in a PAR-Q detailing your basic medical status in order to ensure that you are suitable for the exercise programmes being offered and you may be asked to fill in a lifestyle questionnaire. This information is stored securely in digital form and may be printed and stored under lock and key and is not accessed by anybody other than Steph at The Posture Practice (except in rare circumstances where there may be an emergency or danger to life, in which case Steph at The Posture Practice may be ethically obliged to share your information with the relevant authorities).

For all Low Back Pain sessions and occasionally prior to other sessions (stated when booking) you will be asked to provide more in depth information about your medical history, current status and medications - it may be necessary in some circumstances to communicate with your GP/healthcare professional in regards to your medical status/history. All information shared will be treated in strictest confidence, only viewed by Steph at The Posture Practice; and information only ever shared with your GP/healthcare provider with your explicit consent (except in rare circumstances where there may be an emergency or danger to life, in which case Steph at The Posture Practice may be ethically obliged to share your information with the relevant authorities). This information is stored securely in digital form and may be printed and stored under lock and key and is not accessed by anybody other than Steph at The Posture Practice.
The information is used solely for the purposes of communication in relation to your bookings, risk stratification, and towards designing exercise programmes tailored to your needs and abilities. Information will not be used for marketing purposes unless express permission is granted by you.
It is optional to sign up to The Posture Practice online Portal using the online signup form. By signing up, you are agreeing to an online profile being set up on the website. You can request your profile/account to be deleted at any time. Online accounts will be deactivated following the cessation of your bookings with The Posture Practice. Accounts will be dormant but available to reactivate for 6 months from the cessation of your booking, after which time your account / profile will be permanently deleted. Should you then wish to make further bookings and use The Posture Practice online Portal, you will be required to sign up and create a new login / account.

Your information is NEVER shared with anybody else or passed on to any third parties.
Many clients are linked through Facebook, so Facebook messenger is also a common means of contacting you.

Occasionally - with your permission - text/WhatsApp communication may be used to communicate / notify you of any changes to your bookings.

At all times you have a right to be forgotten and can request for your records to be deleted/destroyed. You also have a right to request to view any and all information The Posture Practice stores about you at any times.
